Recovery rarely happens inside one office. We stay at the center of your care, but real progress pulls in physical, occupational, and speech therapists, complementary specialists, and community organizations working alongside us. This page is your directory to the trusted partners we refer you to, and the people and programs that support you and your family beyond our walls.
Guided to your healthcare village
No one heals alone, and no single clinic can offer everything you need. We call the specialists, therapists, and community organizations we work with your healthcare village: people who bring their own expertise while staying connected back to the plan we build together.
We do not provide physical, occupational, speech-language, or art therapy in our own office, and that is by design. We refer you to specialists who focus on exactly that work, every day, so each part of your care comes from someone who knows it best. The same is true for dizziness and balance retraining, tinnitus and bruxism management, headache care, breathing retraining, massage, myofascial decompression, reiki, and performance therapy: we connect you to trusted partners for each one, and we stay involved so your individual overall health status keeps moving forward as one coordinated plan.

For referring providers
If you are a physician, therapist, or care manager referring a patient to Wellness by Physiatry, fax (773) 312-4522 is a direct, working line to our office. Send your referral note along with the patient's relevant history, current medications, and any recent imaging or specialist reports, and we will follow up promptly to coordinate their care.
