This policy covers the Wellness by Physiatry website: what information it collects, how that information is used, and the choices you have. It does not cover clinical or health information you share directly with the clinic, which is handled separately, as explained below.
Information we collect
The only personal information this website collects is what you choose to submit through our appointment request form. That form asks for six things: your name, phone number, email address, preferred location, whether you are a new or existing patient, and a general message about how we can help.
The form itself asks you not to include medical or health details in your message, and we do not collect health information through this website.
Health information and HIPAA
Any clinical or health information you share directly with the clinic, whether by phone, in person, or through our patient portal, is handled under the clinic's own HIPAA policies and procedures, not under this website's privacy policy.
Cookies and analytics
We use Google Analytics, loaded through Google Tag Manager, to understand how the site is used. Analytics tracking defaults to off (Consent Mode v2, denied by default) and only begins if you accept our cookie banner. You can decline, and the site works exactly the same either way. You can change your choice at any time using "Cookie settings" in the footer.
How we use information
We use the information you submit through the appointment request form only to respond to your request and to help schedule your visit. We do not use it for advertising, and we do not sell or rent it to other companies.
Third-party services
This site relies on two outside services. Formspree delivers appointment request form submissions to our clinic by email; Formspree's own privacy policy governs how it handles that transmission. Google Analytics collects anonymized site-usage data, and only after you accept cookies as described above.
Your choices
- You can decline analytics cookies, or change your choice later, without any loss of access to the site.
- You can also control cookies through your browser settings.
- You can ask us to delete information you have submitted through the appointment request form by contacting us using the details below.
