We stay at the center of your care, but real progress pulls in physical, occupational, and speech therapists, complementary specialists, and community organizations working alongside us. This page is your directory, connecting you to the trusted partners we refer you to, and the people and programs that support you and your family beyond our walls. We call the specialists, therapists, and community organizations we work with your healthcare village: people who bring their own expertise while staying connected back to the plan we build together. We do not provide physical, occupational, speech-language, or art therapy in our own office, and that is by design. We refer you to specialists who focus on exactly that work, every day, so each part of your care comes from someone who knows it best. The same is true for dizziness and balance retraining, tinnitus and bruxism management, headache care, breathing retraining, massage, myofascial decompression, reiki, and performance therapy: we connect you to trusted partners for each one, and we stay involved so your individual overall health status keeps moving forward as one coordinated plan. Our network ## Who will you refer me to? Dr. Kopp evaluates what you need, writes your rehabilitation treatment plan, and refers you to trusted partners for the parts of your care we do not provide ourselves. We stay involved throughout, reviewing progress with you and adjusting the plan as your recovery changes. ### Clinical partners - **Schwab Rehabilitation Hospital** - A Chicago rehabilitation hospital with inpatient and outpatient programs, including care for stroke and brain injury. This is where we refer you for physical, occupational, and speech-language therapy. - We refer you when your care plan calls for hands-on therapy, for example to rebuild strength and mobility, work on daily tasks, or address swallowing and communication after a stroke or brain injury. The therapy is coordinated with the care plan you follow with us. - **Dynasplint** - Dynasplint Systems makes adjustable dynamic splints that apply a gentle, sustained stretch to a stiff joint, often while you rest or sleep. - We refer you when spasticity or muscle tightness is limiting your range of motion and positioning, alongside the medical treatment Dr. Kopp provides for spasticity. - **Specialty therapy partners** - Trusted partners for dizziness and balance retraining, tinnitus and bruxism management, headache care, breathing retraining, massage, myofascial decompression, reiki, and performance therapy. - We refer you when one of these is part of your recovery, and we stay in touch with the partner so the work fits your overall plan. ### Community partners - **Adult day care and disability services** - Safe, supervised daytime programs and advocacy support for you and the people caring for you. - We refer you when you need structure and company during the day, or when the people caring for you need time for work and rest. - **Legal services** - Low-cost help with the practical, non-clinical side of recovery, such as benefits and guardianship questions. - We refer you when a legal or paperwork question is getting in the way of your recovery. - **Food pantries** - Reliable food access for you and your family while you focus on healing. - We refer you when getting enough food is a worry, because good nutrition is part of your recovery. - **Casa Esperanza** - Community support for Chicago families. ### Affiliations - **UIC Psychiatry** - An academic affiliation with the University of Illinois Chicago Psychiatry Department, supporting behavioral health care. - We draw on their support when your symptoms call for more specialized attention than we can offer. - **Hobson Institute** - A brain injury-focused affiliation. - **Coming soon: RUSH and Select Rehabilitation Hospital** - We are building new affiliations with RUSH and Select Rehabilitation Hospital to grow your healthcare village even further. ## How do I refer a patient? If you are a physician, therapist, or care manager referring a patient to Wellness by Physiatry, fax **(773) 312-4522** is a direct, working line to our office. Send your referral note along with the patient's relevant history, current medications, and any recent imaging or specialist reports, and we will follow up promptly to coordinate their care.
